How to Order a ScoliScore™ AIS Prognostic Test

If you are a treating physician (or other authorized healthcare provider) and would like to order the ScoliScore Prognostic Test™, please follow the instructions below:

An authorized healthcare provider:

  1. Completes an initial account setup form.
  2. Places an initial order of ScoliScore Prognostic Test™ sample collection kits.
  3. Collects and prepares specimen for submission to Axial Biotech.
  4. Completes a ScoliScore™ Requisition Form and sends the specimen collection and transportation kit to our laboratory.

ScoliScore Prognostic Test™ results are returned via overnight mail within 3 – 4 weeks of receipt of specimen. In addition, we submit the insurance claim to the patient's insurance for reimbursement.
